Below are download links to the intake forms. You’ll need to download or print the appropriate packet(s) and then fill the forms out.
Double-check to make sure everything has been filled, then send the completed forms back to MCRC in an email, envelope (snail-mail), or fax. You can also choose to bring them back to the center in person. Along with the completely filled intake form(s), MCRC will need a copy of the doctor’s prescription for therapy.
If you wish to transmit your Intake Packet via email, please know that the information is not encrypted and could be intercepted/accessed by a third party. Intake Packets can also be turned in to the contact listed below.
